ELIGIBILITY – The Jamba Juice Healthy Habits Kickstart Sweepstakes & Instant Win Game is open to legal residents of the fifty (50) United States and the District of Columbia who are 18 or older.
SWEEPSTAKES PERIOD – Promotion begins on 1/21/14 at 12:01 a.m. Eastern Time (“ET”) and ends on 2/17/14 at 11:59 p.m. ET.
PRIZES – SWEEPSTAKES GRAND PRIZE: One (1) sweepstakes grand prize is available. The sweepstakes grand prize consists of the following elements: (i) Sixty Thousand HawaiianMiles (frequent flyer miles) for each of the sweepstakes grand prize winner and his or her guest (for a total of 120,000 HawaiianMiles), which is the equivalent of two (2) roundtrip Coach Class tickets between any North America Hawaiian Airlines gateway city (JFK, LAX, SFO, SEA, PDX, SMF, OAK, SJC, SAN, PHX, and LAS) and Honolulu, Hawaii (HNL); (ii) three (3) nights’ standard hotel accommodations (single room, double occupancy, room and tax only) for the sweepstakes grand prize winner and his or her guest; (iii) one (1) Wii UTM prize package, which includes one (1) Wii UTM system, one (1) Wii FitTM U game, one (1) Wii Balance BoardTM accessory and one (1) Fit Meter accessory, for sweepstakes grand prize winner only, (iv) a one (1) year supply of Jamba Juice smoothies for sweepstakes grand prize winner only (awarded in the form of a $500 Jamba Juice gift card) for sweepstakes grand prize winner only; and (v) One Thousand U.S. Dollars ($1,000) spending money (awarded in the form of a check) for sweepstakes grand prize winner only. The Approximate Retail Value “ARV” of the sweepstakes grand prize package is $4,998.98.
INSTANT WIN PRIZES: One Hundred and Five (105) instant win prizes are available and shall be made up of the following:
